1. Acceptance of Terms

By installing or using Contact Form Builder – MT, you represent that you have read, understood, and agree to these Terms of Service. If you are accepting on behalf of a business entity, you represent that you have the authority to bind such entity to these Terms.

2. App Description & Services

Contact Form Builder – MT enables Shopify merchants to build, customize, and embed custom web forms on Shopify storefronts, capture customer submissions, and synchronize submission data to third-party integrations such as Google Sheets, Omnisend, Klaviyo, and Google reCAPTCHA v2.

3. Merchant Responsibilities

As a Merchant using the App, you agree to:

• Maintain the security of your Shopify admin account and credentials.
• Comply with all applicable data protection, privacy, and consumer protection laws when collecting end-user data via forms.
• Provide a clear privacy policy to your storefront visitors detailing how their submitted data is stored and processed.

4. Acceptable Use

You agree not to use Contact Form Builder – MT for any prohibited or unlawful activity, including but not limited to:

• Collecting sensitive personal information (such as credit card numbers, passwords, or government identification numbers) through unencrypted or unauthorized custom form fields.
• Engaging in phishing, spamming, or fraudulent customer data collection.
• Attempting to bypass security measures, disrupt app servers, or reverse engineer any part of the App.

5. Third-Party Integrations

The App connects with third-party platforms including Google Sheets, Omnisend, and Klaviyo. Your use of these third-party integrations is subject to their respective terms of service and privacy policies. We are not responsible for third-party platform availability, API changes, or data practices of third-party providers.

6. Intellectual Property Rights

All intellectual property rights, source code, designs, graphics, and trademarks associated with Contact Form Builder – MT remain our exclusive property. You are granted a limited, non-exclusive, non-transferable, revocable license to use the App solely for your Shopify store operations.

7. Uptime & Service Availability

While we aim to maintain high service reliability and uptime, we do not guarantee uninterrupted, continuous access to the App. We reserve the right to conduct periodic maintenance, perform upgrades, or modify features with or without prior notice.

8. Limitation of Liability

To the maximum extent permitted by applicable law, Contact Form Builder – MT and its developer shall not be liable for any indirect, incidental, consequential, special, or punitive damages, including loss of data, loss of business revenue, or loss of profits arising out of your use or inability to use the App.

9. Indemnification

You agree to indemnify, defend, and hold harmless Contact Form Builder – MT, its owners, and affiliates from any claims, liabilities, losses, damages, or legal fees resulting from your violation of these Terms, infringement of third-party rights, or improper collection of end-user personal data.

10. Termination & Uninstallation

You may terminate these Terms at any time by uninstalling the App from your Shopify admin panel. We reserve the right to suspend or terminate app access immediately if you violate any provision of these Terms.

11. Changes to Terms of Service

We may update these Terms of Service from time to time. Any modifications will be posted on this page with an updated effective date. Your continued use of the App following any changes signifies your acceptance of the updated Terms.
